If you are interested in coaching please fill out a Coaches Application (under resources) for the appropriate age group and send it in with your childs signed waiver.

Thank you so much for taking the time to Volunteer to Coach, we appreciate all of the time and effort that you all put into coaching our kids each year.

INFORMATION

One registration fee cheque can be filled out for all children Registering, see attach price list. However, each child (except U5 & U6) will need a separate UNIFORM DEPOSIT cheque in the amount of $45.00 each, post dated for April 1st, 2011.

All cheques are payable to CVSA.

A CVSA authorized stamp & signature is required by the Government of Canada for the new Child Fitness Tax rebate, PLEASE leave ALL COPIES attached, a stamped copy will be available the following year.

REGISTRATION INFORMATION

THE DEADLINE FOR THE EARLY REGISTRATION DISCOUNT IS JUNE 15, 2011


After June 15th, 2011, the registration fee will increase by $25.00 per person.  Any registrations completed after July 15 will be accepted on a WAIT LIST basis.  While we try to make every effort to place children on teams, we cannot guarantee placement after July 15.

REFUNDS

NEW REFUND RULES: It is required by the Government of Canada that no refund be issued without the original registration receipt, you must return your registration receipt in order to receive a refund.
ALL REFUNDS ARE SUBJECT TO AN ADMINISTRATION FEE - ABSOLUTELY NO REFUNDS WILL BE ISSUED WITHOUT A RECEIPT.
For refunds please mail your receipt with your request and reason for refund to:

Requests for refunds will be accepted up to and including August 31st, 2011

There will be NO REFUNDS after August 31st, 2010

SPECIAL REQUESTS FOR U6 - U12

CVSA is committed to a balanced and FAIR PLAY approach, SPECIFIC COACH requests may not necessarily be accommodated. However, we do encourage you to request to play with other children at these ages and the coordinators try to go out of their way to accommodate as many requests as possible.
